ELIGIBILITY CRITERIA

  • Minimum of 18 years of age.
  • Ability to function independently in daily living tasks.
  • A completed Medical History Statement (DCC-29). A review of the information within DART Cherry’s ‘Program Medical Services’ link for general medical guidelines for participation and admission to the DART Cherry program.
  • Questions relating to medical and/or mental health issues should be addressed with DART Cherry staff prior to transport. Diagnosed mental health conditions or medical issues must be stabilized on medication such that the referral may interact and communicate with others. Any concerns for acute withdrawal will be assessed by medical staff who will disposition accordingly.
  • Referrals on prescription medications must arrive at the facility with a 90-day supply of all prescription medication.
  • A current written TASC assessment and referral that confirms a substance use disorder as defined by the Diagnostic and Statistical Manual, 5th Edition (DSM-5) and ASAM Level III placement.
  • A signed Court Order or Post Release Supervision and Parole Commission Order with a referral to the DART Cherry program.

INELIGIBILTY CRITERIA

  • Cannot have a chronic history of assaultive and violent behaviors.
  • Cannot have pre-existing appointments with primary physician or other healthcare provider.
  • Cannot have medical needs that require a high degree of medical management beyond the scope of practice in the facility including but are not limited to:
    • Seizure disorders
    • Brittle diabetes
    • Uncontrolled hypertension
    • Dementia
    • Schizophrenia
    • Severe Migraines
    • Dialysis
    • Chemotherapy/Radiation
  • Cannot have a current Narcotic and/or Opiate prescription except for active buprenorphine (Suboxone) or naltrexone treatment.
